Mild
Able to perform most activities of daily living

Very Mild
Able to perform most activities of daily living

Some Discomfort
Pain is present but minor.

Bearable
Pain is pronounced but not to the point that you can't make accommodations to deal with it.
Moderate
Still able to perform some activities of daily living, but must make adjustments in order to remain independent.  Inability to manage the pain.
4
Bothersome
Pain is now present and you are constantly aware of it.
5
Very Bothersome
Pain is now affecting your activities of daily living and your ability to function.
6
Terrible
Unrelenting pain that begins to take over your senses and ability to think in a clear and concise manner.
Severe
Unable to engage in normal activities.
Patient is disabled and unable to function independently.
7
Really Terrible
Pain is now controlling your senses; you are unable to do normal activities and may be unable to function without outside help.
8
Brutal
Pain is now to the point where you are no longer thinking clearly and need professional help.
9
Unendurable
Pain can no longer be tolerated.
10
Worst Ever
Generally unconsciousness will result at this level.
